District offers multi week evidence based health programs to residents. These innovative efforts are designed to enhance diabetes and chronic disease management. The Live Well Chronic Disease Self-Management program is designed to assist older adults with ongoing health conditions who want to take charge of their health problems. The Live Well Diabetes Self-Management program helps people with diabetes and pre-diabetes learn the skills they need to manage diabetes and lower their risk for complications. Programs are offered periodically.
