Community health care facility offers primary outpatient care to children and adults of all ages regardless of ability to pay. Services include prenatal care, pediatrics, OB/GYN (for Norwalk residents only), physical examinations, adult medicine, ongoing care for chronic problems, immunizations, same day visits for acute problems, and specialty clinics for diabetes, infectious disease, and asthma. HUSKY eligibility and case management services provided on-site. Pharmacy on-site for patients who need medication. Services are provided regardless of insurance status. Staff will assess income and assets and offer a sliding fee schedule discount based on ability to pay. Patients are expected to pay their portion of the bill at the time of service.
